Eye Examination

 

A comprehensive eye health examination and visual analysis will be performed to determine the visual state of your eyes and to investigate any other signs of disease or other eye health problems that may have not yet become apparent to you. Other items of importance to bring are a list of current medications and current Health Insurance Card. In order for us to provide the best possible eye examination experience, please complete the "Welcome" medical information form and bring it to our practice (or you can download it from our website). This diagnostic information will help us determine your specific needs and aid us in developing your treatment plan and recommendations year after year.
